Work in Action: Quality Improvement Model to Reduce Hospital Readmissions by Addressing Social Determinants of Health

An unprecedented level of urgency regarding equity issues is spurring conversations across society, and healthcare is no exception. There’s an awareness of the need to address social determinants of health, but implementing initiatives and programs that have concrete results in patient health and are easily scalable remain a challenge. This session will discuss how to design a transitions of care model to address gaps in care, inequalities, and social determinants of health. Leave this session with five key interventions to improve outcomes in behavioral health communities and identify gaps that contribute to unplanned readmissions in economically disadvantaged and vulnerable behavioral health patient populations.
