Improving Blood Pressure Compliance in the Ambulatory Care Services (ACS) Setting: A Quality Improvement (QI) Bundle Approach
A poster presentation describing the interventions implemented to improve hypertension for patients in a primary clinic setting.Problem:The high prevalence of uncontrolled hypertension in underserved populations is a major cause of health disparities in the United States and requires innovative healthcare delivery interventions (Pasha et al., 2021). The HTN compliance rate in a Texas public health clinic was 54.2%, below the national benchmark of 76%.
Measurement: Tools utilized include run charts.
Analysis: Statistical Analysis utilized include Tableau Dashboard.
Implementation: PI bundle
Clinical Decision Support: Enhance HTN documentation in EPIC.
Clinical Quality Analytics: Analyzing data derived from the Business Intelligence (BI) report and the Tableau Dashboard.
Focused Audits: Conducting HEDIS (CBP) focus audits.
Education: HEDIS (CBP) compliance, HEDIS (CBP) focus audit data analytics and best practices.
Results/Discussion: The PI bundle significantly improved the HEDIS (CBP) quality metric for adult patients 18 - 85 years of age diagnosed with HTN in the ambulatory care setting.
