Learning Lab: Aggregation – Squeezing the Most Value Out of Your RCAs and FMEAs

As quality professionals, we understand the importance of “sample size” – the more samples we have the better our data and decision-making is. Unfortunately, some of our most important data that deals with some of the most significant risks to the organization has the smallest sample size. Yes, we are talking about our root cause analyses (RCAs) and our failure mode and effects analyses (FMEAs) – both key analyses that help our leadership deal with their most pressing challenges. So what to do? The key is to move beyond looking at each RCA and FMEA individually. It is difficult to make a compelling case to leadership with just one data point. If we want to really squeeze as much decision support value out of these analyses as we can, we need to aggregate, aggregate, aggregate!

In this Learning Lab, we will discuss how to Aggregate your RCAs and FMEAs starting with some simple coding and analysis to open up our understanding of the organizational issues that may be even bigger than those identified in the individual RCAs and FMEAs.

  • Opening the window to “The World of Why”
  • The value of Aggregation.
  • Common Cause analysis – review the basics of coding and analysis. You can do it!
  • Is it legal to mix RCAs and FMEAs together? Oh my!
  • Lead the Pack: Taking it to the next level. Think about your “Integrated Risk Profile”.
  • What you can do tomorrow
